Thank you for choosing Diamond Creek for your dog’s play care needs. We are confident that you will find our facility and services to be truly unique!
We want to make your evaluation go smoothly and start your experience at Diamond Creek off on the right paw. Your evaluation will be done in two parts, the details of which can be viewed on pages 12-13 of the registration packet. These are the forms your evaluator will use to assess your dog. During the evaluation we will also perform a thorough wellness check on your pet to establish a base-line of their physical appearance.
Please download and complete the Doggie Play Care Registration Packet prior to Part 1 of your evaluation so you can fill out the forms ahead of time and we can use your entire appointment to get to know you and your pet(s). We also ask that you bring a copy of your dog’s most recent vaccination history that we may keep on file for future reference. Our Play Care guests must be current on the following vaccinations: Rabies, DHPP and Bordatella. Your vet may fax this information to us at 860-491-3906 prior to your evaluation. Following Part 1 of their evaluation, all approved dogs will need to have a stool sample negative for parasites prior to entering our Play Care environment for Part 2 of the evaluation.
Please contact our office to schedule your appointment for Part 1 of your dog’s evaluation which will take approximately 30 minutes and to pick up a Registration Packet or have one emailed to you to print at home. Dogs approved after Part 1 may schedule Part 2 of their evaluations on Play Care days at 50% of the daily rate so we can observe your dog in a actual play environment.
